Annual Membership
Because we care about our patients, we developed the In-House Membership Plan available at Parkville Pediatric and Family Dentistry. Many people find themselves without any dental insurance coverage these days, and our team can empathize! Our program assists patients who have no dental insurance coverage in making their dental care more affordable.
For a reasonable annual fee, the Membership Plan includes routine dental checkups and x-rays and provides significant savings for some of our most popular cosmetic and restorative services without insurance headaches. Because this plan is not insurance, there are no claim forms, no deductibles, no hidden fees or financial surprises, no required pre-authorizations or third-party interference to postpone treatment, no pre-existing condition limits and no waiting periods—benefits can be used immediately!
How much does the membership cost?
Adults and children $399 per year (valued at $600 in checkups alone!)
What the plan includes:
Adults (18 & Older)
2 preventive dental cleanings
2 exams
One full-mouth set of x-rays as needed or once every 5 years
Necessary bitewing x-rays up to twice per year
Children
2 preventive dental cleanings
2 exams
2 fluoride treatments
Necessary bitewing x-rays twice per year
Panoramic x-ray every once every 5 years as needed
Adults & Children
1 emergency exam & periapical x-ray per year
All other dental care including crowns, resins, pulp treatments, sealants, deep cleanings, space maintainers, bridges, implant crowns and extractions: fees reduced 20% when paying in full the day of treatment by using check or cash.
All other dental care including crowns, resins, pulp treatments, sealants, deep cleanings, space maintainers, bridges, implant crowns and extractions: fees reduced 15% when paying in full the day of treatment by using a credit card.
Member responsibilities include:
Membership fee must be paid up front, in full, and is valid for 12 calendar months.
Membership fee is nonrefundable and cannot be canceled following payment.
Treatment fees must be paid in full on day of service to receive membership discounts. There are no exceptions.
Cancellation or failure to keep a scheduled dental cleaning appointment without the required 48-hour notice will count as one of the two annual checkups and cannot be made up.
Limitations & exclusions of the plan include but are not limited to:
Any treatment which cannot safely be performed due to the general health limitations of the eligible Plan Member as indicated by assessment of our dentists or said Member’s personal physician or physicians.
Any dental procedure considered experimental.
Dispensing of drugs, sales of dental products: MI Paste, Prevident toothpaste, Opalescence whitening to go trays.
Any treatment paid for by Worker’s Compensation, employer’s liability laws, a federal or state government agency, or other insurance coverage carried by the Plan Member.
Any administration of general anesthesia, oral conscious sedation, or nitrous oxide.
Any dental care in which you have been referred to a specialist by Dr.Graue
Procedures resulting from military service or any act of war, declared or undeclared.
Any dental treatment that began prior to the Plan Member’s effective date for eligibility of Plan benefits, including but not limited to teeth prepared for crowns, root canals in progress, or orthodontic services.
Any fees related to admission, use, or stays in a hospital, outpatient surgery center, or similar care facility.
Any procedure that has a poor probability for success based on condition of tooth, teeth, or surrounding structures in the professional opinion of the dentist or specialist OR any procedure inconsistent with generally accepted standards for dentistry.
Extraction of asymptomatic teeth or teeth showing no signs of infection, including but not limited to the removal of third molars (wisdom teeth). (Unless prescribed by an orthodontist.)
Additional exclusions and treatment fees are subject to change without notice.
If you acquire and use ANY 3rd party insurance plan during the 12-month contract period, this membership cannot be used and is non-refundable.
Periodontal Disease Deep Cleanings (Scaling and Root Planning or SRP) are not included in the Dental Plan. If required, they are assessed separately and qualify for the fee reduction. A Prophylaxis (routine preventative cleaning) cannot be performed on a member with periodontal disease, as its whole “purpose” is to PREVENT periodontal disease. If you are diagnosed with periodontal disease, you will need either Deep Cleanings (SRP) or Periodontal Maintenance. Periodontal Maintenance cleanings will be credited with the value of two preventative cleanings per year. If a deep cleaning, SRP is required a 20% discounted fee will be honored.
Maximum annual benefit (maximum write-off) provided is $5000.00 a calendar year. • Not valid with any other offers, promotions, or dental insurance.
The in-House Membership Plan is only valid at Parkville Pediatric and Family Dentistry in Parkville, Missouri.
This plan is non-refundable and non-cancellable and is only valid for 12 months during regular business hours at Parkville Pediatric and Family Dentistry.
Parkville Pediatric and Family Dentistry has the right to refuse service to any Plan Member who fails to follow a prescribed course of treatment by Dr. Graue.
